ABOUT THE ROLE Your focus as Team Leader will be to support the home management team in delivering high-quality care with a difference by providing a safe,
caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff. Other responsibilities will include: Leading in the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans. Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate. Leading with monitoring the well-being of each resident, being aware of any change impacting care and care planning and ensuring that changing needs are met and written records are amended to reflect any variation. Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff., As a Team Leader at Avery Healthcare, located at Acer Court Care Home in Nuthall, Nottinghamshire, you will take on a pivotal role in supporting the home management team. Your core responsibility is to ensure the delivery of high-quality care by creating a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff. This role involves leading the person-centred care planning process, which includes implementation, monitoring, monthly evaluations, care plan audits, and developing and executing action plans. Additionally, you will assist in facilitating the smooth transition of residents in and out of the care home, maintaining clear and effective communication with residents' next of kin, when appropriate. Monitoring each resident's well-being is crucial, ensuring that care plans are updated to address any changes or needs that arise. Practicing safe work systems, particularly in handling residents, is a key aspect of this role,
ensuring the safety of all involved, including residents, visitors, and staff.
