Your focus as Team Leader will be to support the home management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open,
supportive, and inclusive environment for residents, visitors, and staff. Other responsibilities will include: Leading in the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans., As a Team Leader at Avery Healthcare, your focus will be supporting the home management team to deliver high-quality care in a safe and inclusive environment. Your daily responsibilities will encompass leading the person-centred care planning process, conducting monthly evaluations, and developing action plans. You will also assist with smooth transition discharges, monitor and update residents' care plans, and ensure a safe working environment.
