Full Job Description

  • Safeguard the health, wellbeing, and rights of adults we support at all times.
  • Lead and support the delivery of care in line with personalised care plans and risk assessments.
  • Conduct informative shift handovers and ensure continuity of care.
  • Ensure appropriate equipment, medications, and personal support items are available and in use during community access or appointments.
  • Allocate daily tasks to support staff and monitor the quality of their work.
  • Support the completion and review of support plans, health action plans, and risk assessments within agreed timescales.
  • Accurately complete and contribute to reports for reviews, multidisciplinary team meetings, and audits.
  • Liaise effectively with professionals, families, advocates, and stakeholders involved in each individual's care.
  • Coordinate appointments, activities, and personal routines in a person-centred manner.
  • Ensure individuals are treated with dignity, respect, and privacy at all times.
  • Support with personal care, mobility, medication, and delegated health tasks as appropriate to the role and training.
  • Support the smooth daily operation of the service, including taking lead responsibility in the absence of management.
  • Participate in and encourage all relevant training and development.
  • Foster positive relationships with individuals, families, and other professionals.
  • Maintain accurate, timely, and person-centred documentation in line with regulatory and organisational expectations.
  • Promote and ensure health and safety compliance across the service.
  • Participate in infection prevention control and cleanliness routines.
  • Encourage participation in meaningful daily activities, both in and outside of the service.
  • Promote socially inclusive behaviour and support with skill-building towards independence.
  • Provide companionship and support during hospital stays or overnight care needs, where appropriate.
  • Attend and contribute to staff meetings and supervisions.
  • Ensure continuity of medical supplies and liaise with GPs, pharmacies, and healthcare professionals as needed.
  • Complete daily medication audits and ensure safe administration and storage.
  • Undertake security and safety checks, particularly during night shifts.
  • Contribute to service improvement initiatives and regulatory inspection preparation.
  • Supervise, mentor, and guide support staff including performance reviews and monthly supervisions.

    A genuine passion for delivering high-quality adult care.
  • NVQ Level 3 Diploma in Health & Social Care (Adults) or equivalent.
  • Strong interpersonal and communication skills.
  • Ability to work collaboratively as part of a team and independently when required.
  • Proven leadership skills and ability to support others effectively.
  • Ability to remain calm and solution-focused in challenging situations.
  • Understanding of adult safeguarding, mental capacity, and person-centred care principles.
  • Substantial experience working in adult residential or supported living services.
  • Experience supporting individuals with complex needs, learning disabilities, autism, or physical health conditions is desirable.
  • Experience in a senior or shift leading role
