We have an exciting opportunity to join our Children's Physiotherapy Team as a Referral to Discharge Coordinator, playing a vital role in supporting the delivery of high-quality paediatric physiotherapy services across MPFT. This is a key position within the team, responsible for coordinating the patient journey from referral through to discharge, ensuring children and families receive a responsive, efficient and compassionate service. Working closely with the Paediatric Physiotherapy Team Leads, Physiotherapists, Physiotherapy Assistants and the wider multidisciplinary team, you will help ensure that patients are seen at the right time, in the right place, by the right clinician., You will:
- Manage and coordinate the paediatric physiotherapy referral process from receipt to discharge.
- Monitor waiting lists and clinic capacity to support achievement of service targets.
- Schedule appointments and ensure urgent referrals are prioritised appropriately.
- Act as a key communication point between families, clinicians, secondary care providers and external agencies.
- Maintain clinical records within the RiO patient management system.
- Support service improvement projects and contribute ideas to enhance patient pathways.
- Provide day-to-day supervision, guidance and training to administration colleagues.
- Handle patient enquiries and resolve issues promptly and professionally.
- Support clinicians by ensuring clinics run efficiently and available capacity is fully utilised.
