Tameside - Adults Social Worker

• Conducting assessments of individuals' needs and strengths to develop personalized care plans.
• Providing ongoing support and advocacy to service users, ensuring their rights and preferences are respected.
• Collaborating with healthcare professionals, community organizations, and other stakeholders to facilitate effective service delivery.
• Monitoring and reviewing care plans to ensure they remain relevant and effective.
• Maintaining accurate and up-to-date records in compliance with relevant legislation and policies.
  
  To be successful in this role, you must possess the following essential qualifications and skills:
• A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
• Registration with Social Work England.
• Proven experience in adult social work, particularly in mental health settings.
• Strong assessment and analytical skills, with the ability to develop effective care plans.
• Excellent communication and interpersonal skills, with a compassionate approach to service users.
