Full Job Description
The professional positions advertised have arisen as a result of investment in and development of our service in Practical Advice and Support Service. The post involves supporting adults in their own homes to live as independently as possible through meaningful activity. The service is fast paced and supports customers short term. Our customers may have a Learning Disability, Autism, Mental health condition, physical and cognitive impairment. The care team works closely with our well established Multi-Disciplinary Team members including Social Workers, Therapists, and Community Nurses. The Support Worker plays a vital role in delivering high-quality, person-centred care and support to customers with a range of complex health and social needs. The role focuses on maintaining and improving customers' wellbeing, promoting independence, and enabling individuals to live as fulfilling and independent a life as possible. Working as part of a dedicated team, you will provide practical, emotional and psychological support tailored to each customer's needs. This may include personal care, assistance with basic health needs, meal preparation, household tasks, maintaining a safe home environment, community engagement, travel support, and shopping assistance. You may also support customers to follow therapy plans and use innovative assistive technologies, with full training provided. Our team is committed to delivering an outstanding customer experience, providing compassionate support during what can often be a challenging time for customers and their families or carers. These principal responsibilities are not an exhaustive list of duties, only a general guide to the role. In consultation with the post holder, the duties may change from time to time to reflect the changing needs of the customer/service.

- £13.45 per hour in line with the Foundation Living Way
- 32 hours per week available
- We pay for your DBS check
- Designated rota patterns including days, evenings, weekends and bank holidays
