Full Job Description
We are looking for a dedicated Support Worker to join our 7-day hospital discharge service in Sandwell. You'll work closely with hospital teams and partner organisations to help people return home safely after a hospital stay and maintain their independence.
Full UK manual driving licence held for a minimum of 2 years and a willingness to drive service vehicles
Hours: 37 per week (5 days over a 7 day rolling rota. The service is opened 7 days week from 9am - 8 pm so the rota and shifts will be given in advance but they will be expected to work one full weekend in three)
Contract: Fixed term contract until 31st December 2027
