Full Job Description

An exciting opportunity has become available to work within the Care4CE Mental Health Reablement team in Crewe. There are 2 permanent FTE (37hr) Grade 6 Senior Support Worker positions. The team primarily work Monday to Friday 9am - 5pm, although some early evenings may be required. The service is based at Delamere House in Crewe but covers the south of Cheshire East. About the role: The Mental Health Reablement team provides short term (up to 6 weeks) practical support to adults who are experiencing mental health issues and/or drug and alcohol related issues. The successful candidate will be expected to have a caseload and provide one-to-one support in the community. This may include support with debt management/budgeting, housing issues, education, employment, social inclusion, leisure activities, etc. The support focuses on coping techniques, increasing self-esteem and confidence, promoting social inclusion and goal setting to support people to maximise their independence.   • Provide high-quality person-centred support.
  • Work independently in the community and manage their own caseload.
  • Have excellent communication, organisation and record-keeping skills.
