Join Our Team as a Support Facilitator (Home Care) - Make a Real Difference Every Day! Are you passionate about helping people live their best lives? Staffordshire County Council's Provider Services team is looking for dedicated Support Facilitators to deliver person-centred reablement care in the community. You'll be part of an innovative and empowering service that supports individuals to thrive in their own homes. Our service runs 7:00 AM to 10:00 PM, and we need team members who can start early and work flexibly to meet the needs of those we support. Applicants must live within 30 minutes of our base location ST5 1BL to ensure timely response and continuity of care. This role requires travelling within the community, primarily Newcastle under Lyme Borough, and so a full UK driving licence and access to your own vehicle is essential. We have a part time position available with a range of shifts between 7am and 10pm, which will include weekends. There is a business need for the early shifts to start at 7am. We require our team to work flexibly, which means you may be required to work other days to complete training., Working flexibly across various locations in North and East of Staffordshire, you will provide an excellent level of care to enable the people we support to have a comfortable and dignified life within their own homes. As a Support Worker, you will:
- Provide person-centred reablement care in people's homes, respecting dignity and promoting independence.
- Support individuals with daily living tasks, personal care, and medication administration.
- Communicate effectively with individuals, families, and colleagues to build positive relationships.
- Respond flexibly to changing needs and embrace digital tools to improve service delivery.
- Maintain accurate records and comply with health, safety, and safeguarding standards.
- Undertake manual handling tasks as required.
- Drive (using your own vehicle) to deliver care across the local area.
- To complete interventions using Assistive Technology Please note: Due to recent changes in legislation (effective 22 July 2025), this role is not eligible for sponsorship.
- You'll bring a caring and compassionate nature, with a genuine desire to make a positive difference in people's lives every day.
- You'll have strong communication skills and the ability to build trust and positive relationships with individuals, families, and colleagues.
- You'll demonstrate flexibility and resilience, adapting to changing needs and working patterns while maintaining a positive outlook.
- You'll bring practical skills and reliability, including a full driving licence, physical ability for care tasks, and a commitment to learning and development. You'll have:
- Experienced in social care (NVQ2 Provided).
- A driver with access to a vehicle for work purposes.
- Caring, patient, and committed to empowering people to live independently.
- Able to work flexibly across days, evenings, weekends, and bank holidays.
- Physically able to support personal care and manual handling tasks.
- A great communicator with a positive outlook and ability to stay calm under pressure.
- Living within 30 minutes of our base location.

- BLOCK pay, meaning you will be paid for the whole shift duration, including travel time
- For travel, we will reimburse you at £.45/mile, paid from Newcastle Library.
- These positions are predominantly based with the Borough of Newcastle under Lyme area but there is an expectation to travel across the North/East of Staffordshire to support other areas to suit business need.
