Full Job Description

The role of Student Community Public Health Practitioner is a development post building on existing knowledge, skills and competencies to achieve a higher level of practice resulting in an academic qualification as well as a recordable/registrable qualification with the Nursing and Midwifery Council.You will be employed by the London Borough of Sutton but will work as part of the Sutton Health and Care Alliance (SHC).,

  • The assessment and reassessment of care needs, whilst developing, implementing and evaluating programmes of care in conjunction with and under the supervision of the Practice Teacher/Mentor
  • To regularly take part under supervision, in managing a corporate caseload, in order to develop the required managerial skills, including the allocation of work, supervision and support of junior staff.
  • Delivering service activities and manage caseloads to ensure intended outcomes for clients and the Council are delivered within agreed service standards.
  • Participate in and develop health promotion activities in line with Borough priorities.
  • Identify and implement improvements in your own work area in order to deliver continuous service improvement and improved outcomes to clients.
  • Liaise with clients to review service requirements and resolve problems.
  • Keep up to date with changes in policy/legislation/contractual requirements in order to ensure service delivery is effective and complies with appropriate regulations.
