Full Job Description
This is an exciting opportunity to contribute to service transformation and help shape the future delivery of rehabilitation within community hospitals. You will play a key role in demonstrating how therapeutic intervention can improve patient outcomes, reduce deconditioning, enhance patient experience, and support timely discharge and patient flow as part of the Discharge to Assess pathways.
- Deliver timely Physiotherapy assessment and intervention
- Support the implementation and evaluation of innovative rehabilitation pathways designed to promote earlier intervention from therapy services.
- Contribute to the collection and analysis of outcome measures that demonstrate the impact of therapy on patient recovery, independence, and length of stay.
- Facilitate person-centred rehabilitation programmes that focus on individual strengths, goals, and meaningful activity.
- Work proactively to prevent and reduce deconditioning through early mobilisation, functional rehabilitation, and promotion of independence.
- Collaborate with MDT to identify opportunities for service improvement and enhanced patient flow.
- Support patients and families in planning for discharge and ongoing rehabilitation needs.

Are you an innovative Physiotherapist with a passion for improving rehabilitation outcomes and driving new ways of working? We are seeking a motivated and forward-thinking Physiotherapist with experience to join our community hospital team to support the development and evaluation of Discharge to Assess therapy interventions. This exciting role will focus on demonstrating the impact of earlier therapy involvement, reducing hospital-associated deconditioning, and delivering person-centred rehabilitation that enables patients to achieve their goals and return home sooner. Working collaboratively within a multidisciplinary team and wider community services, you will help shape and implement evidence-based approaches that maximise patient independence, improve functional outcomes, and support effective patient flow across the healthcare system.
