Staff Nurse, Swift Ward

An exciting job opportunity has arisen for a compassionate and enthusiastic registered nurse to join the team on swift ward at our St Peters site. Swift ward is a care of the elderly ward within the medical division., 1. Be responsible and accountable for the assessment, planning, implementation and evaluation of individual care needs for assigned patients, ensuring safe practice and maintaining high standards of
2. Promote and provide health education for patients and their carers
3. Establish and maintain effective communication links with other members of the multi-disciplinary team, patients and carers
4. Support NIC and help to facilitate safe discharges for patients, thus reducing the length of stay in hospital wherever possible.
5. Delivery of safe patient care according to the prescribed medication.

Swift ward is a 26 bedded ward caring for older age adults. The ward has a strong MDT and Allied Health Professional team to support with care of our patients, and has close links with the Dementia team.
