Staff Nurse

We are currently recruiting a Staff Nurse to join our Frailty Unit, where we provide specialist care and assessment for older adults living with frailty, complex health needs, and multiple long-term conditions. The multidisciplinary team works collaboratively to deliver person-centred care, promote independence, support safe discharge planning, and improve patient outcomes. To assess, plan, implement and evaluate individualised, person-centred care for patients living with frailty, multiple comorbidities and complex health needs within your sphere of responsibility. To support the effective management and organisation of work within the Frailty Unit, ensuring patients receive safe, timely and coordinated care through multidisciplinary team working. To act at all times in accordance with the NMC Code, carrying out duties in line with Trust policies, procedures and professional standards. To contribute to comprehensive frailty assessments, care planning and discharge planning, promoting independence and supporting patients to achieve their optimum level of wellbeing. To work collaboratively with families, carers and the wider multidisciplinary team to ensure holistic care and smooth transitions between hospital and community services. To recognise and respond appropriately to the changing needs of frail older adults, escalating concerns and contributing to the prevention of deterioration where possible. To maintain accurate and contemporaneous patient records and communicate effectively with all members of the healthcare team. To participate in service development, quality improvement initiatives, training and continuing professional development to enhance the delivery of frailty care. Ensure all records relating to patient care are accurate, clear, concise, legible and in accordance with the NMC Standards for Records and Record Keeping. 3. Actively assess and manage risk within the clinical area. 4. Ensure adherence to the Trust Medicines Policy and the NMC Standards for the Administration of Medicines. 5. Participate in audits of practice. 6. Participate in the teaching and supervision of other members of staff under the direction of the ward manager. 7. Support the Clinical Governance systems to delivering high quality of care. 8. Assist the ward manager in achieving budget control and order supplies when necessary. 9. Participate in personal development reviews for junior staff. 10. Working in accordance with the NMC code of professional conduct. Competently undertake expanded role. 11. Participate in benchmarking activity within the Clinical Area and implement outcome from Essence of Care. Administrative/Managerial 1. Take charge of a shift and ensure organisation and delivery of patient care. 2. Adhere to the Bed Management policy and adherence to the Admission and Discharge policies. 3. Support the ward manager in effective Resource Management. 4. Participate in ensuring the delivery of the Trust targets. Human Resource Management 1. Participate in the training and development of student nurses and other staff as appropriate. 2. Participate in clinical supervision for junior staff in accordance with Trust policy. 3. Assist the ward manager to ensure that there is appropriate allocation of staff duties. 4. Ensure communication of information of a complex nature is given sensitivity and confidentiality is maintained.

1. Possess expert clinical knowledge appropriate to area of responsibility. 2. Frequently manage and participate in distressing situations. 3. Use professional judgement and initiative to promote safe practice. 4. Demonstrate knowledge of staff and resource management.
