Full Job Description

Assess patients' wellbeing and related needs, identifying and escalating physical or psychological health concerns to the appropriate team members.
Plan, deliver, and evaluate individualised programmes of care, working collaboratively with patients whenever possible.
Monitor patients' mental and physical wellbeing and contribute to safeguarding individuals whose health or safety may be at risk, in line with local policies such as the Observation Policy and Risk Management Policy.
Establish and maintain effective communication with the multidisciplinary team, service users, and carers regarding care plans, daily activities, and ongoing interventions.
Maintain patient confidentiality at all times in accordance with the Data Protection Act and professional codes of conduct.
Involve service users, carers, and relatives throughout episodes of care wherever feasible, ensuring choices are offered and respected.
Ensure support workers complete delegated ward tasks and undertake patient observations in line with policy and expected standards.
Contribute to service development and support positive change initiatives that enhance the quality of care provided on the ward.
