Band 5 Staff Nurse Short Term Care Beds Working as a Staff Nurse for Leeds Community Healthcare NHS Trust within the Short Term Care Beds service is very different from many traditional nursing roles. We work as a community within a community, providing person-centred care and rehabilitation to support patients on their recovery journey. The Short Term Care Beds service provides care for patients who require a short-term stay following discharge from hospital. While returning home is the preferred outcome for many patients, some require a temporary stay in a supportive, homely environment where 24-hour care is available. The service supports patients to regain independence, achieve their rehabilitation goals, and plan for a safe discharge either to their own home or, where appropriate, to a nursing or residential care setting. Our aim is to work in partnership with patients and their families to help them achieve their recovery goals. This is delivered through a multidisciplinary
team approach, with support from Registered Nurses, Health Care Support Workers, Occupational Therapists, Physiotherapists, Speech and Language Therapists, Dietitians, and General Practitioners (GPs). The Short Term Care Beds service operates 24 hours a day, 365 days a year, and you will be required to support patients across a variety of shift patterns to ensure the continuous delivery of high-quality care and rehabilitation services., Community nursing in the recovery hubs suits people who can work independently and within the wider multi-disciplinary team. Youll be resilient, motivated, and adaptable to working in an ever-changing environment and of course, a passion for providing the best possible care. You will have great communication skills and maintain a high standard of clinical work when carrying out nursing procedures. You will ensure compliance with our clinical governance framework, you will motivate colleagues, champion service user safety and dignity and drive continuous improvement. The post holder will be responsible for their designated workload within the bed base and will have access to support from a senior clinician The post holder will be responsible for the assessment of patients care needs and the development, implementation, and evaluation of programs of patient care working within an integrated health and social care Team For more Information about this Job, please contact - Paula Simpson
