Full Job Description
The Wolds is seeking a committed and enthusiastic Band 5 Registered Mental Health Nurse (RMHN) to join our expanding team.
This is a unique opportunity to work within a pioneering Reablement Service, supporting patients to regain skills, confidence, and independence as they prepare for a successful transition back into the community.

You will hold responsibility for a small, manageable caseload, working closely with your Associate Nurse and wider multi-disciplinary team (MDT). Our approach is highly collaborative, and the expectation is that all staff maintain an understanding of each patient's recovery journey, needs, and discharge pathway.
This is a full-time post with hours worked on a rotational basis, including nights, weekends, and bank holidays. Appropriate shift enhancements apply in line with Agenda for Change., Developing and maintaining collaborative, person-centred care plans
Completing and updating all relevant clinical documentation
Safe medication management
Active participation in MDT meetings and handovers
Acting as an advocate for patients and supporting their recovery goals
Facilitating ward-based groups and therapeutic activities
