Within the Midlands Partnership Foundation Trust (MPFT) we have an exciting opportunity for a Band 6 Senior Staff Nurse to join our rapid Response Community Nursing Team in the East staffs locality. We are looking for a dedicated and compassionate nurse to further enhance the high quality patient experience we offer in the community.
The successful applicant needs to have a commitment to providing care in patients' own homes and residential homes providing acute care including IV antibiotics, line care, palliative care inclusive of EOL care and syringe driver and symptom management, Tissue viability and pressure area care, diabetes care, continence care and managing long term conditions, being able to work on their own initiatives an work well within a team demonstrating these wide range of clinical skills.
Community services are managing patients with more complex acuity than ever before with increasingly specialist treatment and care being provided by highly skilled staff. For the right applicant training and development can be provided to meet the individualised needs of patients in our community along with enhancing skills and knowledge for the successful candidate including leadership skills.

Main duties of the job
The Adult community Service provides care to patients in their own homes and is pivotal in the prevention agenda. You will deliver a high quality service, working closely with GP's and a multidisciplinary teams in addressing a range of care issues across a spectrum of health conditions and preventing avoidable hospital admissions.
We are a friendly and innovative service covering Burton upon Trent and surrounding villages As a service we have a great interest in moving Community Nursing forward in line with local population and service needs.
You must be a qualified nursing practitioner with current NMC registration. Previous nursing experience, able to demonstrate a commitment to delivering high quality targeted personal care and hold a full driving licence.
You must be able to demonstrate leadership skills and be able to make sound clinical decisions independently. You will be expected to be a positive, influential role model, supporting both staff and service delivery.
Appointment to a development role is with the understanding that the successful candidate holds the Practice Assessor or equivalent mentor course/certification and has completed or currently undertaking relevant post qualification accredited study.
Post holders will be required to work flexibly on a rota system covering a 24 hours service across a 7 day week., + Be competent as appropriate to the requirements of the role.

+ Be a Registered Nurse and have a detailed professional and clinical knowledge. They will be a focus in the following areas;
+ IV antibiotics
+ Line Care
+ Palliative care
+ Management of continence
+ Administration of medication
+ Management of long term conditions
+ Be responsible for a defined population of service users in a variety of settings across the Partnership Trust including the community and hospitals.
+ Be skilled in the assessment of complex/specialist care/support needs and in the development, implementation and evaluation of care/support packages.
+ Have supervisory, training and leadership responsibilities where appropriate and will have some responsibility for team performance and service developments/improvements.
+ Demonstrate initiative and be creative in finding solutions to problems.
+ Work with the Operational Lead and Lead Specialist Practitioner undertaking delegated tasks in team management.
