Full Job Description

The advertised post is for a full time Specialty doctor in Medicine for Older People and Rehabilitation. The post holder will be working for Hampshire and IOW Foundation Trust (HIOWFT). The Portsmouth and South East Hampshire (PSEH) Division provides community care across our PSEH geography. This comprises inpatient and community-based rehabilitation services, community nursing and urgent response services, and specialist medical services. This post is to provide medical support for inpatient rehabilitation wards.

This new post will provide medical cover to our ward at Petersfield Community Hospital and to provide absence cover for other wards within our geographical footprint. The work involves admitting and completing medical assessments of patients as they are admitted to the ward, identifying and managing their medical problems, providing proactive care of chronic health conditions and responding to changes in their health.

Main duties of the job

The post holder main responsibilities will be to work as a ward doctor as part of the PCH MDT team overseen by a Consultant Physician Geriatrician along-side two advanced clinical practitioners, AHPs and experienced ward nursing teams. The postholder will be based on the two rehabilitation/physical health wards: Rohan and Cedar.

The role will include medical assessment and triage of medical admissions, along with ongoing management. You will take a lead role in the clinical assessment of patients who have chronic disease and multi-morbidity presentations, including screening patients for disease risk factors and early signs of illness, making a differential diagnosis and prescribing treatments.

Management of patients will involve admission assessments, organising investigations and treatment planning for patients with a range of illnesses and complex rehabilitation needs. The post holder will be expected to familiarise themselves with the local arrangements for requesting radiological and pathological investigations as well as recording of medical notes and communicating with GPs and other community staff. Full training for the local, relevant, information systems will be given. RiO is the Clinical IT system used both for in-patients and community activity. A trust laptop comes with the post ensuring your access to the Trust, Ward, local Acute Trust and Hampshire electronic records and also all Trust training resources, library, intranet and email services.
