We want you to be involved with the development of the frailty service at Rochdale Infirmary, supporting residents of Heywood, Middleton, and Rochdale (HMR)., As a full-time Band 7 therapist specialising in frailty, you will work with doctors, advanced practitioners, nurses, therapists and pharmacists to support frail patients both in the hospital and in the community. Join the Frailty Team as a Specialist Therapist Are you passionate about community/hospital dedicated to the care of frail patients, promoting a home first approach. We're seeking a dynamic therapist at Rochdale Infirmary to support residents of Heywood, Middleton, and Rochdale (HMR). You'll play a key role in delivering the frailty service, ensuring timely, appropriate care is provided in patients' own homes. You'll work closely with patients and carers to manage health needs, promote self-care, and develop personalized plans. Collaborating with multidisciplinary teams, you'll help shape high-quality, patient-centered services aligned with trust goals.
