Specialist Support Worker - Complex Care Team

• Provide specialist, person-centred support to people with complex needs, which may include autism, learning disabilities and physical disabilities.
• Put the individual at the heart of everything you do, supporting people to make choices, achieve their goals and have greater control over their lives through personalised support plans.
• Use and develop your specialist skills through hands-on experience and ongoing clinical and specialist training, including supporting with medication, health needs, medical equipment and behaviours that challenge.
• Support people access their local community, maintain relationships, enjoy leisure and social activities and take positive risks.
• Be part of a skilled and supportive team, sharing your knowledge, contributing to personalised planning and working alongside colleagues to deliver safe, consistent and high-quality support.
