Are you passionate about delivering exceptional, specialist care to people living with Parkinsons Disease? If so, this could be the perfect opportunity for you., The successful candidate will be responsible for: Managing an active caseload of people with Parkinsons Disease, throughout their disease pathway. Working with patients and their carers to provide interventions and to develop self-management plans to optimise health and quality of life and reduce avoidable hospital admission. Working in collaboration with patients and their carers, GPs & Hospital Consultants to achieve optimum therapeutic benefit from medication and treatment regimes. Supporting the multidisciplinary team to provide specialist care to patients with Parkinsons Disease and be an integral part of the Community Neuro Rehabilitation Team and Neuro Specialist Team at First Community. Developing and delivering education and training relating to specialist area - for the MDT, for other health and social care professionals and for people affected by their condition and carers.

We are seeking a dynamic, motivated, and forward-thinking Registered Nurse or Allied Health Professional (AHP) to join our dedicated Neuro Specialist Team. This is a rewarding community-based role where youll make a real differencesupporting patients, their families, and carers to live well with Parkinsons. Were looking for someone who is passionate about patient-centred care and long-term condition management with strong organisational and clinical decision-making skills. You must be able to communicate with confidence and compassion and be someone who thrives working both independently and as part of a team To be eligible for this post you must:

  • Be a Registered Nurse or Allied Health Professional eligible to become an Independent Prescriber (if the qualification is not already held)
  • Hold a full UK driving licence and have access to a vehicle for work purposes
