As we launch this exciting new service, we are looking for a Specialist Physiotherapist to join our growing team and play a key role in supporting people to recover, regain independence and achieve the outcomes that matter most to them.This is an exciting opportunity to join a service at the beginning of its journey. Working alongside colleagues across health and social care, you will help develop and deliver high-quality rehabilitation and reablement pathways that support a Home First approach, improve patient outcomes and enable people to live as independently as possible within their own homes and communities. You will work collaboratively with a multidisciplinary team to develop personalized rehabilitation plans for adults with diverse physical and neurological conditions.
