Are you an experienced Physiotherapist looking for an opportunity to help shape a brand new service and make a lasting difference to people across Medway?HCRG Care Group is proud to have been chosen by Medway Council to deliver the Intermediate Care and Reablement Service from 1 October. As we launch this exciting new service, we are looking for a Specialist Physiotherapist to join our growing team and play a key role in supporting people to recover, regain independence and achieve the outcomes that matter most to them.This is an exciting opportunity to join a service at the beginning of its journey. Working alongside colleagues across health and social care, you will help develop and deliver high-quality rehabilitation and reablement pathways that support a Home First approach, improve patient outcomes and enable people to live as independently as possible within their own homes and communities. helping people maximise their independence following illness, injury or hospi, The Specialist Physiotherapist role at HCRG Care Group involves working within a transformative service launched by Medway Council to offer Intermediate Care and Reablement. This permanent position based in Sittingbourne focuses on supporting individuals' recovery and independence. You will work collaboratively with a multidisciplinary team to develop personalized rehabilitation plans for adults with diverse physical and neurological conditions.
