Clare Lodge is a national provider of secure accommodation "welfare" only placements and the only all female unit in England. The home is governed by Peterborough City Council. CPFT provides an in reach health service that includes caring for young people's physical and emotional wellbeing. The team also uses the integrated framework for secure care and works in a collaborative way with PCC to support a therapeutic environment. Clare Lodge In Reach Health Team provides:

• Trauma informed delivery of health care.
• Comprehensive holistic health assessment and intervention including mental health, neurodevelopmental, speech and language, sexual health, and physical health.
• Direct therapeutic interventions.
• Drug and alcohol advice.
• Outreach service.
• Named health professional role and opportunity to liaise with national professional network.
• Group work.
• Occupational Therapy Input.
• Integrated working alongside Clare Lodge residential and education teams.
The Post Holder will be part of the multi-disciplinary psychology Led Healthcare In-Reach team to provide evidence based physical health assessment and treatment to young people aged between 12-17 in a secure welfare setting, as part of a six day a week service. The postholder will assertively engage with young people through direct work as well as with the young person's local and wider professional network to identify and manage physical health needs of the young person. The postholder will act as the health professional responsible in ensuring good physical health for the young person and improving future long term health outcome. Our clinical teams deliver many NHS services, not only via inpatient and primary care settings, but also within the community. These services include children's, adult and older people's mental health, forensic and specialist mental health, learning disabilities, primary care and liaison psychiatry, substance misuse, social care, research and development., 1. To provide physical nursing assessment and treatment to young people within a psychology led inreach health team daily. 2. Complete standardised specialist health assessments for children in secure care including information gathering from health agencies who have previously worked with young person. 3. Work within own competencies to refer young people to appropriate specialist services, including universal public health services. 4. To provide practical and emotional support to encourage parents and residential staff to take a proactive approach to the physical health needs of the young person. 5. To provide practical and emotional support to encourage young people to take a proactive approach to their own physical health care. 6. To work in partnership with parents and residential staff within the individual psychological formulation for the young person 7. To lead on sharing holistic health needs to professional meetings centred around the young person 8. To lead on comprehensive health care plans developed in partnership with the young person which will include mental health, physical health and emotional wellbeing. 9. Maintain accurate and comprehensive clinical records and notes and complete reports to a high standard. 10. Direct work with the young person to promote independence, develop life skills, and support safe transition from Clare Lodge. 11. Identifying and referring to appropriate health services as part of the young person's transition from Clare Lodge into a community setting.
