Full Job Description
As a Specialist Palliative Care Nurse, you will play a vital role in supporting patients and those important to them at some of the most significant moments of their lives. You will deliver high-quality, evidence-based specialist palliative care, providing expert clinical advice, advocacy, and compassionate support to patients, families, and colleagues across seven days of service delivery.
Working as part of a highly skilled multidisciplinary team, you will help ensure care is person-centred, dignified, and responsive to individual needs, while promoting comfort, choice, and quality of life.
The team works closely with wards, departments, and services across the Trust, as well as with partners beyond organisational boundaries, to ensure seamless and coordinated care. You will also contribute to the education and development of staff, supporting excellence in palliative and end-of-life care across the organisation.

You will be encouraged and supported in your professional development, with opportunities to grow your specialist knowledge and skills within this rewarding role., As a Specialist Palliative Care Nurse, you will manage a caseload of patients with complex palliative care needs, working closely with their families and carers to deliver high-quality, person-centred care. You will undertake holistic assessment, care planning, and evaluation, supporting patients across hospital and community settings.
Using your specialist knowledge, you will provide expert advice and support to patients, carers, and health and social care professionals. You will work collaboratively with the Specialist Palliative Care team and wider MDT, addressing complex physical, psychosocial, and ethical aspects of care. Skilled communication and advocacy are central to the role, supporting individuals through sensitive and challenging situations.
The role requires flexibility, including unsocial hours, to meet the needs of a seven-day specialist palliative care service.
Key responsibilities include:
+ Managing a caseload of patients and families/carers with specialist palliative care needs
+ Providing expert advice on symptom control, psychosocial support, and ethical issues
+ Developing and maintaining therapeutic relationships through skilled, compassionate communication
+ Acting as an advocate for patients and families
+ Supporting patients towards self-care and educating carers
+ Working collaboratively across health and social care settings
