Full Job Description

We are seeking a highly skilled and compassionate Integrated Specialist Palliative Care Clinical Nurse Specialist (CNS) to join our multidisciplinary team. This is an exciting opportunity to deliver expert palliative and end-of-life care across acute, community and integrated settings, supporting patients with complex needs and their families., To provide a highly specialist palliative care service for people with life-threatening illness, aiming to enhance quality of life for patients and carers with complex physical, psychological and emotional needs. To manage a caseload and assess and evaluate the care given. To act as a specialist resource, to other professionals including independent contractors, this includes the development and delivery of educational programmes both on a primary care and the private sector (care homes) related to the care and management of patients.

  • Registered Nurse status with significant post-registration experience
  • Specialist knowledge and experience in palliative and end-of-life care
  • Excellent communication, leadership and multidisciplinary working skills
  • Commitment to education, service development and quality improvement
