Specialist Occupational Therapist - (Virtual Ward)

You will be a member of a dynamic multi-disciplinary team providing Urgent Community Response and Discharge to Assess interventions in people's homes. Working with a well-resourced, multi-disciplinary team you will be supported to work in a blurred boundary way, and develop new clinical skills.

It is essential that you are a car driver and have a Full UK Driving License, reasonable adjustments will be considered.

This post provides a fantastic opportunity for a driven, passionate individual to contribute to the development of new pathways in delirium and falls - alongside the day to day work of preventing hospital admission and providing right time, right place, assessments in the community.
