Specialist Occupational Therapist

Join our Merton Enhanced Home-Based Rehabilitation Team (MEHBRT) as a Specialist Occupational Therapist and help people make meaningful progress towards recovery and independence in their own homes. Working as part of a multidisciplinary team, you will providetimely, person-centred occupational therapy to patients transitioning from hospital who have ongoing health or social care needs. This is an opportunity to combine specialist clinical practice with leadership and service development. You will:

• Assess patients and develop rehabilitation plans that reflect their individual needs and goals.
• Support the development,deliveryand evaluation of the MEHBRT service.
• Manage your own caseload, including complex cases, with access to senior support when needed.
• Provide clinical leadership, advice, education and supervision to colleagues and students.
• Contribute to service quality,performanceand continuous improvement.
You will work collaboratively with patients, families,carersand colleagues across the multidisciplinary team to support safe, effective rehabilitation and positive outcomes. You will provide specialist assessment,rehabilitationand clinical leadership within the MEHBRT.
• Assessing referrals from hospitals for patients who are ready for discharge but require ongoing health or social care support.
• Managing an individual caseload, including patients with complex needs, seeking senior support whereappropriate.
• Developing,implementingand evaluating occupational therapy interventions that promote independence and support safe transitions home.
• Contributing to the development and ongoing evaluation of the MEHBRT service.
• Providing clinical leadership and advice to Physiotherapists, Rehabilitation SupportWorkersand the wider multidisciplinary team.
• Providing education and supervision to Rehabilitation Support Workers and multidisciplinary students.
• Line managing and supervising junior staff within the service.
• Supporting the team to achieve agreed performance indicators andmaintainhigh standardsof quality and patient care.
• Working collaboratively across professional teams to ensure a coordinated and seamless service for patients.
You will use your clinicalexpertise, professionaljudgementand communication skills to support positive outcomes for patients and contribute to the continued development of the service.
