Full Job Description

In this role, you will work within the Occupational Therapy Memory Service Team, contributing specialist assessment and intervention as part of a wider multidisciplinary team. The service applies the Cognitive Disabilities Model in practice, supporting individuals living in the community who are experiencing changes in cognitive functioning, including mild cognitive impairment (MCI) and dementia, with the aim of promoting independence and maintaining functional ability., As a Band 6 Occupational Therapist, you will:

Conduct comprehensive OT assessments to inform intervention plans, identify strengths, and develop adaptive strategies.

Play a vital role in maintaining independence, promote self-management, and support inclusion.

Actively contribute to risk assessment, ensuring effective risk management in collaboration with the MDT.

Provide an occupational perspective within MDT care plans.
