Specialist Nurse / PT / OT Discharge Coordinator

An exciting opportunity has arisen for a motivated and experienced Discharge Coordinatorto join our dynamic Hospital Discharge Team.You will play a key role in supporting patient flow and coordinating safe, timely dischargesfrom admission through to discharge. Managing a caseload of patients with varying levels ofcomplexity, you will work closely with multidisciplinary teams, community services, socialcare, and external partners to remove barriers to discharge and reduce length of stay.The successful candidate will lead on complex discharge planning, support pathwaycoordination, facilitate access to specialist equipment and community services, and act as akey escalation point to ensure patients are discharged safely and without unnecessary delay.You will also provide leadership and support to discharge coordinators, promote best practice,and contribute to service improvement initiatives.This is an excellent opportunity for a skilled professional with strong clinical

Main duties of the job

o In liaison with ward staff, community, social services and other healthcarecolleagues facilitate timely, appropriate and safe discharge of patients. o Attend Daily Reason to Reside Meetings, MDT meetings, Professional andfamily meetings, Daily Board rounds on allocated wards. o Ensure that the Early Discharge Notification (EDN) to be completed within o 24 hours of hospital admission, Needs Based Assessment (NBA) and anyother onward referrals are completed in a timely manner o Ensure the person and any carer's, family or friends are involved in andinformed about all aspects of the discharge process o To promote and ensure that early bed release guidelines are adhered to;ensuring that all appropriate patients are moved to the discharge lounge o Promote nurse led discharges at weekends and assist on competencylevels to empower staff both at ward level and within the discharge team o Actively support and facilitate the transfer of patients who are privatelyfunding their care to
prevent extended length of stay. To adhere to the Nationaland Trust Policy for Discharge to family and carers within the agreed time frameto avoid extended length of stay. o Work flexibly in shift patterns to meet the needs of the Trust, includingbank holidays, unsocial hours, cross-site working and weekend working
