Specialist Nurse - Health Inclusion Team

An exciting opportunity has become available for a Specialist Nurse with the Health Inclusion Team for 28.5 hrs. We are looking for a strong team player, who can work on their own initiative and provide a person-centred approach to patients independence and well-being. KEY RESULT AREAS:

To support UK new entrants, register with their preferred choice of local GP

TB screening and Blood Bourne Virus screening, adults and children

Referrals and signposting to other services

Attend the multi-agency drop-in at Hope House and community events, The health inclusion team aims to support individuals who are socially excluded, who typically experience multiple interacting risk factors for poor health. This includes facilitating the integration of overseas arrivals into mainstream health services and address inequalities with health and wellbeing within the borough of Barnsley.

The Health Inclusion Team is part of the local outreach team working in partnership with Barnsley Councils Housing Service and local substance misuse service, working directly on the streets to locate, engage with and support individuals that are homeless, have been homeless or are at risk of becoming homeless.

The service also provides a weekly wound care drop-in clinic for individuals that are living street-based lifestyles, run in collaboration with Waythrough's Barnsley Recovery Steps service. At the clinic wounds are assessed with basic treatment and self-care advice provided.

COMMUNICATION WITH OTHERS:

Health Inclusion Team provide face to face support at the womens migration group, community halls of Roma gypsy travellers sites to provide a cohesive and collaborative effective service.

Attend MDT and Safeguarding meetings as a professional advocate for the trust and patients we care for.

Attend the Homeless and Inclusion Health Network

Self-motivated and confident in analysing issues, implementing effective solutions independently.
