Full Job Description

We are looking for a compassionate and skilledSpecialist Nurseto lead the delivery of specialist nursing care for children and young people with life-limiting conditions and their families. This is an opportunity to make a meaningful difference by providing expert symptom management, hands-on nursing care, and emotional and practical support. You will help children and young people receive personalised, family-centred care, including supporting families to care for their child at home wherever possible. You will provide day-to-day leadership for the Kaleidoscope Team and work closely with Children's Community Nursing, hospitals, hospices, social care and other partners to coordinate safe and effective care. The role offers a balance of clinical practice, leadership, service development and education. You will contribute to improving children's palliative and end-of-life care while supporting colleagues, parents and carers. Participation in the 24/7 end-of-life on-call rota will be required when needed. We particularly welcome applications from people from under-represented backgrounds. Key responsibilities include:

  • Providing specialist nursing care, symptom management and end-of-life support for children receiving care at home, working closely with the Children's Community Nursing Team.
  • Assessing referrals and eligibility, including nursing and Continuing Care assessments.
  • Delivering hands-on clinical care and supporting the safe administration and management of controlled drugs in the community.
  • Providing day-to-day leadership and support to the Kaleidoscope Nurse.
  • Working with Play Specialists, social care, hospitals, hospices and other professionals to coordinate care and support transition to adult services.
  • Leading and contributing to safeguarding processes, including attendance at relevant meetings and case conferences.
  • Maintaining accurate clinical records, contributing to service reporting and supporting delivery against service requirements.
  • Delivering training and education for parents and carers.
  • Practising in line with the NMC Code, CLCH policies and professional standards, and participating in clinical supervision and relevant meetings.
  • Promoting evidence-based practice through audit, guideline development and service and practice improvement.
