Full Job Description
We have an exciting opportunity for a permanent Band 6 Specialist Nurse for Children's Continuing Care. The post holder will be responsible for coordinating and managing a caseload of children and young people with complex health needs. The role focuses on delivering high-quality, child-centred care through comprehensive assessment, care planning, and multi-agency working, ensuring care packages are safe, effective, and aligned with national Continuing Care guidance.,
- Undertake comprehensive Continuing Care assessments and reviews in line with national frameworks
- Coordinate and manage complex caseloads, ensuring timely follow-up and case oversight
- Develop, implement, and evaluate individualised care packages in partnership with families and the MDT
- Liaise with the ICB, education, social care, and other agencies to ensure integrated care delivery
- Support complex discharge planning and transitions of care
- Provide clinical leadership and guidance within the team
- Contribute to training, supervision, and service development activities
- Maintain accurate documentation and ensure compliance with governance and safeguarding standards
