Full Job Description
The Family Hubs Team in Calderdale is an evolving multi-disciplinary service offer, complementing the Calderdale 0-19 Service and the wider Children's Services offer. The post holder will have established links across the Family Hub network and other Calderdale partners. They will be responsible for delivering specialist support and advocacy for children and young people in KS3 and KS4 who are not regularly attending school., Responsible for delivering specialist support and advocacy for children and young people in KS3 and KS4 who are not regularly attending school. This includes children and young people who are exhibiting emotionally based school avoidance (EBSA), those who are persistently absent, those at risk of exclusion, and those who are electively home educated (EHE). A key element of the role is to gather data on this cohort of young people to inform and support development of service where required. Responsible for working across the local authority and health services using a collaborative approach; working with a wide range of professionals and services to ensure young people are supported to attend school regularly, and where they are electively home educated, that they can access health and wellbeing support. Co-ordinate and facilitate access to a broad range of health services, reflecting the physical, behavioural, emotional, social, and psychological needs of young people - including childhood immunisations, PSHE curriculum topics, health needs assessment and supported access to specialist services where indicated. Develop therapeutic relationships with young people and work in complex family situations to improve outcomes for young people. Deliver interventions based on best practice and within governance frameworks. To gather qualitative and quantitative data to inform an improved understanding of the cohort, including an action plan to develop service.
