Full Job Description
We are seeking a compassionate and forward-thinking individual to join our Adult Community Service team as a Specialist Doctor and clinically lead the expansion of our Hospital at Home programme.
This pivotal role focuses on delivering holistic, patient-centred care in the community, reducing hospital admissions, and enhancing integrated working across acute, primary, and social care settings. You will collaborate with multidisciplinary teams to provide high-quality medical care at home, ensuring patients receive timely support in the most appropriate setting.
Key Responsibilities:
Lead the Hospital at Home programme, supporting step-up and step-down care
Provide clinical assessment, diagnosis, and treatment of acute and chronic conditions
Coordinate with ED, inpatient wards, radiology, and primary care teams to ensure seamless care
Develop personalised care and prevention plans, including Respect plans
Educate patients and families to promote self-management and health awareness
Supervise and mentor junior staff and advanced care practitioners
Contribute to quality improvement, clinical governance, and strategic planning, Lead the Hospital at Home programme, supporting step-up and step-down care
