Full Job Description

An exciting opportunity for a Specialist Dietitian to work with our friendly dietetic team at Warrington Hospital within the North Cheshire and Mersey NHS Foundation Trust.

The post holder will provide dietetic advice for people living with Diabetes. The post will be based at Warrington Hospital with clinics in the community and Warrington or Halton Hospital.

The successful applicant will contributed to structured group education with the diabetes nursing team in addition to face to face and telephone clinics to ensure the patients across the Warrington and Halton Boroughs have access to evidence based, holistic, diverse advice to support them with the long term self management of diabetes.

The department provides student training placements and there is scope for the successful applicant to contribute significantly to this. There will also be opportunities to develop supervisory skills through supporting band 5 Dietitians and dietetic assistants.

You will be involved in the development, implementation and audit of patient care practices as part of clinical governance. We promote continuous professional development and have good access to IT and library facilities., To work to a high standard as an autonomous Dietitian. To provide a range of specialist assessments, diagnosis and dietary treatments to patients presenting with a variety of conditions, with a focus on long term conditions, primarily diabetes and others such as hyperlipemia, reactive hypoglycaemia and other endocrine conditions.

This post will combine outpatient dietetic led clinics, MDT clinics and diabetes group education with the goal being to provide evidence based education, SMART care plans and health goals.

Excellent communication and interpersonal skills are essential, as you will work closely with diabetes consultants, specialist nurses and greater MDT to ensure education is fully integrated within patient care pathways. You will also liaise closely with the acute and community services which operate within the North Cheshire and Mersey NHS Foundation Trust footprint.
