Full Job Description
You will work as an independent practitioner delivering specialist dietary advice, structured education, and personalised care for people with type 1, type 2, and gestational diabetes across Berkshire. The role offers excellent opportunities to expand your clinical expertise in areas such as carbohydrate counting, insulin management, and behaviour change. If you are passionate about improving outcomes for people with diabetes and enjoy working collaboratively with a wide range of professionals, we would look forward to hearing from you.,
- Deliver specialist dietetic assessment, care planning, and interventions for patients with diabetes across community and hospital settings
- Provide evidence-based dietary advice for type 1, type 2, and gestational diabetes
- Plan and deliver structured education programmes for patients and healthcare professionals
- Work as part of a multidisciplinary team including consultants, nurses, GPs, and community staff
- Support behaviour change through effective communication, counselling techniques, and patient-centred care
- Develop and review patient information resources and education materials
- Participate in audit, service evaluation, and quality improvement initiatives
- Provide training and support to student dietitians and other healthcare professionals
- Manage and prioritise your own caseload while maintaining accurate clinical records
