Full Job Description

This is an exciting opportunity for a dynamic, professional and enthusiastic team member to work autonomously with a stroke and neuro-rehabilitation caseload. You will be joining a well-established MDT which offers community stroke and neurological rehabilitation pathways. We provide services in client's homes and other community settings.
You will take an active role in team and service meetings so contributing to the positive development of the team, and assist in the supervision of other Physiotherapists and support workers., To provide a specialist patient centred physiotherapy service to those who are referred to the community stroke and neuro rehabilitation service.
To undertake the assessment and clinical diagnosis of neurologically impaired patients using investigative and analytical skills; formulating an individual management and/or treatment programme according to the patients goals using advanced clinical reasoning and a range of treatment skills, including knowledge of alternative treatment options.
To undertake the continuous assessment and evaluation of treatment programmes to ensure maximum patient benefit, and modify them according to the patient's needs.
To assess and manage clinical risk and accept clinical responsibility as an autonomous practitioner for a designated caseload of patients, and organise this efficiently and effectively with regard to clinical priorities and use of time.
To utilise complex communication skills to be able to overcome barriers to communication.
Provide advice and education to patients and carers in conjunction with the multi-disciplinary team and signpost to appropriate agencies and charities.
To be aware of, and comply with, organisational and departmental policies and procedures and to be involved in reviewing and up-dating as appropriate.
To maintain own continuing professional development and develop current knowledge of evidenced based practise.
