Full Job Description
We are looking for a pharmacist with a proven track record supporting acutely unwell medical and surgical patients, to work in the Virtual Hospital in West Essex.
The team cares for acutely unwell patients in homes. We enable patients to remain at home by preventing unnecessary admissions to hospital and by supporting them to leave hospital earlier to continue their treatment at home. This includes the utilisation of remote technology to provide a safe alternative to hospital admission.
The post holder will provide specialist clinical pharmacy support to the Virtual Hospital, to deliver medicines-related care of patients, support the provision of medicines management services and participate in training and education. This is an exciting opportunity to shape services that will improve the experience of healthcare for the local population and deliver an NHS national priority.

The post includes participation in the weekend and bank holiday dispensary rota, plus responsible pharmacist and on call rota and there may be opportunity to work additional hours elsewhere within the pharmacy team, as part of our staff bank., This post is based within the Virtual Hospital MDT that accept referrals from acute hospitals and community providers. We offer expert medical treatment, clinical advice and personalised medication management plans in real time, to keep acutely unwell patients at home or support those patients who are discharged early from hospital.
You will be expected to forge strong professional relationships with nurses, paramedics, allied health professionals and doctors. This role requires a passion for clinical pharmacy and a genuine drive to help patients. You will be working to provide practical clinical solutions, and you will be changing patients' lives for the better. We are looking for an enthusiastic and motivated individual to join the team with a range of skills and clinical expertise. You need to be flexible, have excellent communication skills and appreciate the need for professional development within your practice to ensure we establish a responsive and effective team.
Although based in the Virtual Hospital, this post forms an important pillar of the EPUT pharmacy department. At times, the post holder will need to, supervise junior colleagues. A commitment to providing responsible pharmacist duties plus out of hours services are implicit. Post holders should be registered with the GPhC and be able to demonstrate their clinical credentials.

+ To contribute to the organisation and management of the clinical pharmacy service to the Virtual Hospital.
+ To provide clinical pharmacy and medicines management advice in order to maximise benefit and minimise risk in relation to medicines
+ To support and participate in clinical audit.
