Full Job Description
Would you like to work with patients in their own homes to manage their medicines following discharge from hospital. The Pharmacy Connecting Care team review patients, conduct clinical medicine reviews and carry out interventions where appropriate to improve pharmaceutical care. They will provide medication education, assess adherence and follow up patients as appropriate to monitor outcomes. An opportunity has arisen for an enthusiastic pharmacist to join the Connecting Care Pharmacy team within Mid Yorkshire Hospitals NHS Trust. The team work alongside multi-professional colleagues to improve the care of patients through promoting evidence based and patient-centered care. You will work as part of a multidisciplinary team to enable patients to manage their own medicines safely. You will be part of our progressive pharmacy team and contribute to the implementation of the Medicines Optimisation and Pharmacy Services strategy, striving for excellence to provide the highest standards of care, The Specialist Pharmacist will work closely with the Advanced Clinical Pharmacist to support the delivery of Medicines Optimisation and Pharmacy services to Community Services This includes: Contributing to the development and maintenance of policies and guidelines. Liaising with all grades of medical and nursing staff over relevant aspects of the pharmacy service. Helping to implement necessary service changes as a consequence of these consultations. Identifying, reporting on and responding to risk management issues or clinical incidents, referring to peers as necessary. Generating and reviewing reports in order to assist Advanced Clinical Pharmacists to inform the Trust about medication usage and expenditure and/or safety issues related to medicines. Participating in audits to demonstrate the effectiveness or identify development needs of all systems relating to the use of medicines. Deliver, in collaboration with staff in the Medicines Information department, support for
Formulary & Drug Usage Audit initiatives. Clinical service commitment will consist of community services. Visiting patients in their own homes, or care homes to conduct clinical medication reviews and improve adherence. for our patients. In addition to excellent communication and clinical skills you will have previous experience of working within a clinical environment. You will have completed a post-graduate diploma in clinical pharmacy, be near to completion, or have the equivalent relevant clinical experience and skills to support your work as a specialist clinical pharmacist. Evidence of multi-disciplinary teamwork would be advantageous.
