Specialist Children's Community Nurse

We are seeking to appoint a dynamic and enthusiastic candidate to the role of Specialist Children's Community Nurse, located at Cedar House, within our St. Michael's Community Hospital site within the Specialist Nursing Bereavement & Play Team. The base hours for this role will be Monday - Friday 8:00am - 4:00pm but the formal working pattern will be agreed following appointment.

The specialist children's nurse will support children who have an Enfield based GP with continuing care needs or who are life limited or life threatened conditions and this will include children and young adults who require end of life care. To be a key professional working alongside the multidisciplinary team (MDT), to provide a full range of services for the individual child & family.

To be expert clinical children's nurse to meet the needs of a diverse and challenging group of children and young people. To assess and review children and young adults who are currently receiving or may be eligible for continuing health care funding

The post holder has responsibility for the delivery of the service and for implementing and reviewing systems, policies, standards and procedures that ensure the provision of high-quality services within frameworks agreed by the Trust and Commissioners.

The post holder will ensure the promotion of children's safeguarding and promoting the welfare of children and young people. You will be responsible for monitoring performance activity and implementing service improvement plans

The post holder will be required to travel between Trust sites and partner agency venues and carry out home visits and school visits.

North Mid is part of North Central London integrated care system - consisting of the NHS and Local authority organisations in Camden, Islington, Barnet, Enfield and Haringey. As with other ICS's, we are working increasingly closely with partners and indeed many of our financial and performance objectives are measured at this system level. Whilst all organisations remain as standalone, statutory bodies we have an ICS infrastructure for making shared decisions and agreeing shared approaches.
