Full Job Description
We have an exciting opportunity for an experienced Specialist Child and Adolescent Mental Health Senior Practitioner to join the Complex Health Hub at NNUH. This role forms part of the wider Complex Health Hub which is responsible for Safeguarding, Mental Health, Dementia, Learning Disability and Autism, Substance Misuse, MCA & DoLs, Homelessness and Prison Healthcare. We are a team who all have strong motivations to provide the best care possible for those with mental health needs whilst they are with us within the NNUH, you will have the benefit of being able to work autonomously whilst having a supportive team around you to guide decisions where needed. We pride ourselves on promoting mental health within the wider hospital and best practice for managing complex cases within a busy acute hospital. The post holder will provide advice, guidance and care planning to those patients who require intervention for their Mental Health whilst an inpatient at NNUH. This will include the opportunity to gain experience and knowledge of communicating and working closely with wider system partners, in order for a safe and appropriate discharge for any young person with mental health needs from the hospital. The role will also involve regular contact with system partners to ensure an equitable service for this patient group. The post holder will be expected to Lead the delivery of integrated care planning and person -centred interventions for children and young people up to the age of 25 with an identified mental health need and / or complex psychological needs whilst working alongside the multidisciplinary team in the implementation and review. The post holder will be expected to be able to provide expert advice, guidance and support to staff in how to support the management of high risk patients using least restrictive and compassionate practice, whilst role modelling best practice. The post holder will be expected to work in effective partnership with senior clinicians at NNUH, NSFT, NHSE, ICB, Social Care and CPFT to ensure appropriate pathways for accessing the correct clinical care, in the correct environment for our patients. The applicant will need to be proactive and able to work autonomously within the NNUH, providing advice, guidance and signposting to young people, their families, wider support networks and ward staff.
