37 hours per week x 2 (Community Hospital Beds) 37 hours per week x 1 (Home First Team) This advert is open to both internal and external applicants., The Hospital Social Work Service is a large, dynamic team operating across multiple locations. The service is made up of three integrated teams: Home First, Community Hospital Team, and the Integrated Discharge Hub based at UHNM. Each team plays avital role in supporting timely discharges, promoting independence, and ensuring individuals receive the right care in the most appropriate setting. Working across these areas provides a varied and rewarding experience, with opportunities to develop skills in fast-paced hospital environments and community-based discharge planning., Qualified Social Worker registered with Social Work England Strong assessment, risk management and decision-making skills Able to work confidently in a busy, ever-changing environment Passionate about person-centred, strengths-based practice

Proven ability to complete timely, high-quality Care Act assessments within a hospital setting Experience of working effectively within a multi-disciplinary team, building positive professional relationships Strong knowledge and application of the Mental Capacity Act, safeguarding processes, and risk assessment Ability to manage complex and high-risk cases, making sound, defensible decisions under pressure Commitment to strengths-based, person-centred practice, promoting independence and choice Excellent organisational skills with the ability to prioritise workload and meet competing demands in a fast-paced environment
