Full Job Description
At Leicestershire County Council, we have an exciting opportunity to support hospital discharges as part of the Transforming Care programme. We are looking for a Social Worker who is committed to improving the lives of people with Learning Disabilities, Autism, and Mental Health needs by reducing reliance on inpatient hospital care and enabling individuals to live in the community with the right care and support. The role will involve working collaboratively with health partners on complex hospital discharges. This post is open to experienced Social Workers, and qualified Approved Mental Health Professionals. Applicants should have a passion for working collaboratively with people facing a wide range of challenges. The main base for the role is the Coalville Offices at 3 High Street, Coalville. However, the post will involve travel to hospitals within Leicester and Leicestershire, as well as to out-of-county settings. All our teams are friendly, supportive, and committed to ensuring manageable workloads. You will receive regular supervision, and teams participate in weekly Group Supervision Meetings to promote the best possible outcomes for the people we support across Leicestershire. We provide a comprehensive induction package, along with ongoing access to tailored learning and development opportunities. This includes bespoke continuing professional development sessions and access to academic research and practice expertise to enhance skills, experience, and professional practice. - Have knowledge of social work methodologies, social care policies such as person-centred approaches and asset based social work, and relevant legislation including The Care Act, Mental Health Act, and Mental Capacity Act.
- Have experience of effective assessment and risk management delivery, and multi-agency/multidisciplinary working and/or hospital discharge processes.
- Have experience of partnership working with adults to identify and meet need with creative and pragmatic solutions.
- Be a confident communicator who is skilled in diplomacy, persuasion, assertive and conflict management and able to build effective working relationships.
- Be able to work proactively individually but also as a team member.
- Have good prioritisation and time management skills and have a valid UK driving licence or access to a vehicle In addition, we also expect you to share and to delivering accessible and inclusive services which meet the needs of all of our residents. Salary: Between £32,070 to £42,699 (plus £3,300 market premia payment for Level 3 Social Workers) Approved Mental Health Professionals will be paid at between £43,860 to £47,829 per annum (pro-rata for part-time) (Pay award pending) (plus £3,300 market premia payment)
