Are you passionate about mental health care and looking for an exciting opportunity to join a supportive community mental health team? Look no further! We are currently seeking Social Worker or Occupational Therapist to become part of our dedicated Community Mental Health Team. Based at the Whiteleaf Centre in Aylesbury, our team is committed to providing exceptional care and treatment to adults with mental health challenges. Our goal is to deliver patient-centred care of the highest quality, while supporting our staff in their roles and fostering their professional growth. As part of the Aylesbury CMHT Adult Mental Health Team, you will work alongside a multidisciplinary team that includes medical professionals, psychological therapists, social workers, occupational therapists, RMNs, and support workers. We also collaborate with third-sector workers who specialise in employment, housing, and social needs. To bridge the gap between patients and clinicians, we have also introduced Peer Support Workers who bring the invaluable perspective of lived experience.,

  • Work as an autonomous practitioner within a multi-disciplinary team to perform comprehensive assessments in collaboration with service users and their families/carers.
  • Partner with service users and their families/carers to create and execute treatment plans that ensure safety, promote well-being, and support independence and community inclusion.
  • Provide interventions following NICE guidelines to promote recovery and prevent relapse in various settings, regularly reviewing care and outcomes.
  • Support and supervise junior team members, act as a mentor/preceptor to student nurses/trainees, and participate in clinical education and practice development initiatives.
  • Engage in operational planning, policy implementation, and service development within the team, including leading on delegated projects and participating in research and audit projects
  • Applicants must have a full valid driving licence and have access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required. Please note if you are based outside of the UK you will need to hold SW England or HCPC registration in order to be considered for this role.
