We are seeking a dedicated and compassionate Social Worker to join our team for a 12-month contract. In this role, you will be responsible for conducting comprehensive reviews for individuals eligible for Section 117 aftercare, following their support under the Mental Health Act. • Must be registered with Social Work England
• Must be eligible to work in the UK
• Experience of working with vulnerable adults within a hospital setting preferred
• Experience of report writing; such as social care assessments
• Must have experience of completing Care Act Assessments and support plans
• Experience of investigating and chairing safeguarding meetings preferred
• Experience of managing complex cases
• Experience of liaising with other professionals and agencies
• Experience of arranging safe and timely discharges for service users
• Familiar with self-directed support / personal budgets
• Familiar with CHC and Section 117 eligibility and applying for funding
• Experience of using Liquid Logic would also be beneficial but not essential
