Full Job Description
We are seeking a dedicated and compassionate Social Worker to join our team for a 12-month contract. In this role, you will be responsible for conducting comprehensive reviews for individuals eligible for Section 117 aftercare, following their support under the Mental Health Act. - Must be registered with Social Work England
- Must be eligible to work in the UK
- Experience of working with vulnerable adults within a hospital setting preferred
- Experience of report writing; such as social care assessments
- Must have experience of completing Care Act Assessments and support plans
- Experience of investigating and chairing safeguarding meetings preferred
- Experience of managing complex cases
- Experience of liaising with other professionals and agencies
- Experience of arranging safe and timely discharges for service users
- Familiar with self-directed support / personal budgets
- Familiar with CHC and Section 117 eligibility and applying for funding
- Experience of using Liquid Logic would also be beneficial but not essential

SO2 (£36,124 - £40,476) - starting salary of £38,626 for applicants who have been qualified for two years or more, and have passed their Practice Observation and Progression (POP) assessment' 37 hours, Full-Time Temporary (12 months) Hybrid: Baker Street and homeworking. Expectation that in the office for duty and Team Meetings.
