What youll do

  • Work closely with adults and families to provide tailoredsupport and care planning in a strengths based and person centred way, embracing what community assets and strengths.
  • Championindependence, dignity, and wellbeing in every interaction
  • Collaboratewith health professionals, community partners, and colleagues to deliver joined-up services
  • Bringcompassion, resilience, and creativity to complex situations
  • Help develop our workforce through supervision of up to 4 Community Care Officers.
