Full Job Description
Social Worker As part of this team, you will complete social care assessments, support plans and reviews designed to support the delivery of a coordinated response to individual's needs. The team works in partnership with all internal and external professionals involved in assessment and service provision, e.g. Health, police, housing etc. including Court work when required. You will need to be someone who can provide oversight to members of the team as well as contribute to the provision of high quality assessments, reviews and support plans for individuals. - Deliver a professional social work service in line with statutory responsibilities and organizational policies.
- Promote independence and recovery through strengths-based and outcome-focused service delivery.
- Collaborate with other professionals and agencies to address complex needs and achieve optimal outcomes for individuals and their carers.
- Conduct high-quality assessments and develop support plans in consultation with service users and their carers.
- Ensure safe and timely discharges from hospital settings, supporting individual choice and independence.
- Utilize personal budgets creatively and flexibly to meet the needs of service users and their carers.
- Maintain accurate records using electronic systems and contribute to team initiatives and service development.
- Support and mentor newly qualified or less experienced staff and students.
- Participate in continuous professional development and maintain emotional resilience and professional wellbeing.
Qualifications: SW, CSS, DipSW, or Social Work Degree; current Social Work England registration. - Experience: Accepting referrals, assessing needs, managing caseloads, and working in social services or similar settings.
- Skills and Abilities: Knowledge of relevant legislation, effective communication, IT literacy, organizational skills, and the ability to work independently under pressure.

You will be required to provide a car for use in connection with the duties of this post and must be insured for business use. However, we may consider you if you cannot drive because of a disability.
