Full Job Description
- To provide specialist psychosocial, emotional, and practical support to patients with palliative care needs and their families/carers.
- You'll deliver holistic assessments, care planning, safeguarding and practical/psychosocial interventions, working closely with our MDT and community partners.
- Facilitate access to relevant resources and services, safeguard vulnerable individuals, and contribute to multidisciplinary, person-centred palliative care.
Degree in Social Work (or equivalent) and Social Work England registration. - Post-qualifying experience in adult social work (e.g., hospital, community, mental health, or palliative care).
- Evidence of continuing professional development relevant to palliative/end-of-life care, safeguarding, or adult social care.
- You will be able to bring your exemplary communication skills which will enable you to build positive, trusted relationships with patients and families, ensuring they are central to care delivery and decision making.
- Ability to facilitate advanced care plan conversations.
- Prepared to work in a busy and emotive environment as part of a strong and vibrant multi-disciplinary team.
St Gemma's Hospice is the largest hospice charity in Yorkshire, supporting thousands of patients and families each year. We are a local, independent charity with almost 50 years of expertise in providing specialist end-of-life care to the people of Leeds. The Hospice provides In-Patient care but is not just a building; most of St Gemma's patients are supported in the place they want to stay - often their own homes. Our exceptional team, united by our mission to deliver high-quality care with compassion and dignity, consists of over 300 staff, alongside 800 volunteers who generously and passionately donate their time. Our Academic Unit in Palliative Care is at the forefront of clinical research and training, in collaboration with The University of Leeds. We were the first University Teaching Hospice in the world and have been able to supports research, education and training, and evidence-based practice on a local, national and international scale. Our Family Support Team is responsible for caring for adults with complex palliative care needs and their families. - Professional, St Gemma's Hospice is a Leeds-based charity providing expert care and support for local people with life-limiting and terminal illnesses. We offer care to patients and their carers in the Hospice's In-Patient Unit, through the St Gemma's community team, and via our day services, which include a Day Hospice and Out-Patient Department. St Gemma's welcomes and provides care to everyone regardless of people's age, gender, disability, race, religion, sexual orientation or diagnosis. Our care is always completely free of charge. St Gemma's is the largest hospice in Yorkshire and one of the largest in the UK.
- Generous annual holiday entitlement starting at 28 days plus statutory Bank holidays (FTE). Your entitlement will incrementally increase over time, up to 34 days after 10 years' service. You will also have the option to buy up to 5 days' holiday each year, subject to terms.
