Social Worker Assistant

We're recruiting a Social Work Assistant to join our Assessment, Review and Hospital Discharge Team at Milton Keynes City Council.Our team plays a vital role in supporting adults at key points in their lives, helping people access the right support, maintain their independence and achieve the outcomes that matter most to them. Working across assessment, review and hospital discharge services, you'll make a direct difference to the wellbeing of local residents and their families.This rewarding role involves working closely with adults, carers, health professionals and partner organisations to assess needs, review support arrangements and help people transition safely from hospital back into their communities.
