Full Job Description

This position offers an excellent opportunity to become part of our team and provide support across the organisation. We pay an attractive salary of £40,444 - £43,149 and offer access to training and development opportunities. We are seeking a Social Worker to join our Hospital Discharge Team, supporting Caerphilly residents to achieve safe and timely discharge from hospital. The role sits within the Hospital Discharge Service, part of the wider Community Resource Team (CRT) - a large multi-disciplinary service working alongside OTs, Physios, Pharmacy, Rapid Nursing, and the in house domiciliary care service. This role involves working in a fast-paced hospital environment with health and social care colleagues. The post holder will also be required to work across other hospital sites within and outside Caerphilly Borough, including NHH, RGH and PCH. For the role, we ask that you have:

  • A Diploma / Degree in Social Work or predecessor qualification
  • Experience of work within a hospital/community setting
  • A Full UK Category B (Cars) driving licence and the use of a motor vehicle insured for business purposes to travel throughout the borough to visit clients and attend meetings, We are legally required to ask you to provide evidence of your right to work in the UK. Successful applicants are required to provide appropriate documentation such as a birth certificate, passport or work permit in accordance with the Immigration, Asylum and Nationality Act 2006.

The current registration requirements state that completion of the Consolidation of Practice qualification within your first 3 years of practice is mandatory (if you qualified after 1st April 2016).
